to include most recent clinical documentation and any supporting documents you may have for placement consideration.
Those may include:
- PCP
- Clinical Assessment Addendum (will need to have supporting CCA)
- Any Psychological / Psychiatric Assessments
- Placement History
- IEP (Any of these items if applicable)
- Please indicate Level of Care
